HealthPathways

HealthPathways Sydney is an online information portal designed to help clinicians guide patients through local primary, community and secondary health care systems.

HealthPathways Sydney is the collaboration between Sydney Local Health District (SLHD) and Inner West Sydney Medicare Local (IWSML) with support from the Agency for Clinical Innovation using the HealthPathways model developed in Canterbury, New Zealand.

HealthPathways Sydney heralds a new era in collaboration at a local level between primary and secondary care clinicians and practitioners. In developing the initial wave of local content pathways General Practitioners and private Allied Health providers are collaborating with public health clinicians to remodel service access and identify concise and salient clinical information to enhance both pre referral and long term patient management.

HealthPathways Sydney Program Manager Paul Bennett said, "Sydney Local Health District hosts a large and diverse network of medical services and specialists that can be difficult to navigate. The idea is to give clinicians a tool that will help them identify the very best care options for their patients based on their specific conditions and requirements."

The Program Team have been working with a variety of clinical services to develop the initial waves of pathways. GPs are now able to access clinical and referral information for liver disease, sexual health and ante-natal shared care. These will be shortly followed by pathway sets for:

  • Drug Health
  • Community Child Health
  • Mental Health
  • Orthopaedics

Ahead of the launching of the full website, General Practitioners and private Allied Health personnel were invited to take a sneak peak of the HealthPathways Sydney drafting site for a three week period. The drafting site provides a window to the Pathway format as well as showing what's currently in development.

To celebrate the going live the HealthPathways Sydney Program Team took centre stage at the inaugural IWSML Congress held on March 16th at Concord Medical Education Centre. The theme for the congress being 'Connecting the Community: the many ways journey'.

Alongside the launch presentation by Mr Bennett, Dr Margaret Lynch, GP and Clinical Lead from Hunter New England HealthPathways provided an overview of the benefits of pathways to general practice and patient care that have been demonstrated through the HNE's process evaluation of their implementation of HealthPathways.
