External and Overseas Researchers

For any external researcher / non-SLHD employee seeking authorisation to be on-site in any department or service within the SLHD (RPAH Zone) to access either patients, staff or patients' medical record information, the following documentation must be provided:

• If they are an investigator:
- Their details on the SSA, including whether they are a post-graduate student.
- their signature on the Declaration of Researchers, co-signed by the Principal Investigator to endorse their inclusion.

• If they are a research assistant
- the Principal Investigator must endorse their inclusion in the project by listing their name, role in the project and relevant experience in their cover letter

• If they are a post-graduate student (Masters or PhD candidates) of any Australian University
- Provide evidence of candidature (current enrolment documentation)
- Names of their academic and clinical supervisors

• Evidence of a successful Criminal record Check (CRC)
• Copy of University/Other Institution Photo I.D card
• The researcher’s signature and witnesses’ signature on the Privacy Statement (a witness can be anyone who knows the researcher)
• If a University student, a signed Student Code of Conduct Declaration
• A signed Ministry of Health Code of Conduct form to confirm that they have read and will abide by the NSW Health Code of Conduct

To include new personnel please provide the following:
• A letter signed by the Principal Investigator requesting the addition of the new researchers
• If they are an associate investigator

- Their details, including whether they are a post-graduate student, and their signature on the Declaration of Researchers and co-signed by the Principal Investigator to endorse their inclusion (attached)

• If they are a research assistant
- the Principal Investigator must endorse their inclusion in the project by listing their name, role in the project and relevant experience in their cover letter

• If they are a post-graduate student (Masters or PhD candidates) of any Australian University
- Provide evidence of candidature (current enrolment documentation)
- Names of their academic and clinical supervisors 

 Requirements for the CPC-RPA Clinic
Please liaise with Ms Catherine Yates, Manager of CPC-RPA Clinic, who will advise on the documentation she requires for access to the CPC-RPA Clinic. As per usual procedure the requirements for any other external researcher remain the same (see above):
• Evidence of a successful Criminal record Check (CRC)
• Copy of University/Other Institution Photo I.D card
• The researcher’s signature and witnesses’ signature on the Privacy Undertaking (a witness can be anyone who knows the researcher)
• If a University student, a signed Student Code of Conduct Declaration
• A signed Ministry of Health Code of Conduct form to confirm that they have read and will abide by the NSW Health Code of Conduct


SYDNEY DENTAL HOSPITAL

• Liaise with the SDH General Manager to ensure that they know overseas personnel will be on site and what documentation the GM requires (if any)
- If they apply for an Honorary Position with the SDH a copy of the letter from HR approving this must be provided.
As well as any documentation required by the GM if the researcher has a Sydney University appointment please provide:

• Copy of University appointment letter
• Copy of Photo ID
• Signed Privacy Undertaking
• Signed NSW Health Code of Conduct
• As many research projects at the Dental Hospital include children as participants
- evidence of a successful Working with Children Criminal record Check (CRC) – This must be sought on their arrival and at their own cost.

No new overseas researcher/research assistant will be authorised to work on research projects until all of the above documentation is received and authorised by the Research Governance Office.
