There are many types of services available to meet the needs of older people, people requiring rehabilitation and their carers/families. Most of these services are provided to people living in the community, including care after a person is discharged from hospital.
Each service has different eligibility criteria and availability, just like each individual has a range of unique health and care needs.

To help make it easier for people to work out what services they might be able to access, an ‘intake’ system has been created. This intake system means the person requiring care (or their carer) rings a central number and speaks to a single person who takes down all their details, discusses the full range of possible requirements and then refers the person to the services most appropriate to meet their needs. This saves the caller having to make multiple calls to the same organisation, often repeating the same information over and over again.
