Sydney District Nursing Service brochure for patients

Sydney District Nursing is a key service of Sydney Local Health District. We offer compassionate and comprehensive nursing care in your home or at a Community Health Clinic near to where you live.

We provide services in the eight local government areas within the Sydney Local Health District borders (Canterbury, Strathfield, Burwood, Ashfield, Canada Bay, Leichhardt, Marrickville and Sydney (South)).

We work in close partnership with our hospitals (RPA, Concord, Canterbury and Balmain), our allied health professionals, general practitioners or GPs and community organisations in Sydney.

Our focus is on your independence, while delivering the best clinical care to suit your needs.

We aim to provide excellent healthcare to you in a timely, professional, safe and compassionate way.

This service is provided by Sydney Local Health District as part of the public healthcare system.

Sydney District Nursing Service

We provide expert nursing care in the areas of:

  • Acute and chronic wound care assessment and management
  • Hospital in The Home (HiTH) – for hospital type care in your home including intravenous and subcutaneous medication management – eg/ drips and antibiotics
  • Palliative care including symptom management, end of life care and advanced care planning
  • Continence care assessment and management
  • Care for chronic medical conditions
  • Information on Health Coaching Services
  • Information about and referral to allied health services such as physiotherapy, occupational therapy, podiatry and speech pathology
  • Diabetes education for patients at risk of developing diabetes and newly diagnosed patients

Accessing or booking a Sydney District Nurse

Sydney District Nursing Service

There are four ways to access our services:

  1. Referral by your GP
  2. Referral by the doctor treating you in hospital
  3. Referral by your carer or allied health professional on your behalf
  4. Referral by you (self referral)

 

Referrals and appointments are made through Community Health Access Intake for Nursing (CHAIN) which operates from 8am to 4pm seven days a week, including public holidays.

If you are referred by a doctor to the service, someone from ACT will phone you or your carer within 24 hours to confirm your care arrangements.
