Direct GP referral to HITH
New Referral Pathway to Hospital In The Home, Sydney Local Health District.
Sydney Local Health District, Hospital in the Home (SLHD HITH) service provides acute care to patients who otherwise would require traditional hospital inpatient care. The service is available for selected patients with cellulitis, mastitis, community acquired  pneumonia or Pyelonephritis.

From October 12 direct referrals to SLHD HITH services will be available to all general practitioners in the Sydney LHD, Monday - Friday. Currently there is no weekend referral service available.

Information on how to make a referral can be found on the HealthPathways Sydney website – https://sydney.healthpathways.org.au. Follow the clinical pathways for assessment and required referral information, and for links to all four SLHD HITH services.

SLHD HITH services  operate out of Balmain, Royal Prince Alfred, Concord and Canterbury hospitals.

Balmain HITH service is open 7 days/week, the others are open Monday- Friday.
Patients receive assessment, monitoring and treatments in their home with the Sydney District Nursing Service with medical governance provided by a HITH Doctor.

We advise at this time due to staffing capacity considerations, referrals will only be accepted if safe clinical governance and service provision can be assured.

Balmain Hospital HITH (BH)
