Concord Centre for Mental Health Pharmacy Department is committed to quality improvement. As such, in 2015 an audit was carried out to evaluate Clozapine initiation Monitoring Documentation. Given the high risk nature of this drug and the extensive baseline and ongoing clinical monitoring requirements, patients commenced on clozapine during a one year period were audited retrospectively to see how much of the requirements had evidence of completion. The outcomes of this audit were presented to medical staff and quality improvement strategies were implemented as a result.
