Community Nursing

The Sydney District Nursing Services for older people are generally provided in the home to assist in maintaining or improving independence and to promote good health. Services are available 24 hours per day, seven days per week, depending on individual needs and circumstances. These are determined based on an individual assessment.

Sydney District Nursing services include:
  • Treatment and dressing of wounds;
  • Medication assistance;
  • Management of bladder and bowel continence problems;
  • Support and care when someone is dying;
  • Help with organising personal care assistance;
  • Health promotion 
Referrals for Sydney District Nursing may come from hospitals (on discharge), or from General Practitioners, other community care services, family or individuals.
