Community Based Therapy

A range of specialist therapies are available in community health centres and in people’s homes. Service availability varies according to location, but may include assessment and treatment by a team consisting of:

  • Physiotherapists;
  • Occupational therapists;
  • Speech therapists;
  • Nurses;
  • Nutritionists;
  • Social workers; and
  • Podiatrists.
 
People are usually referred to therapy services by their GP or by a health service provider when they are being discharged from hospital.
 
Apart from therapy programs offered by Aged Chronic Care and Rehabilitation Services, most hospitals also provide a range of therapy services that older people may be referred to. Contact your local hospital
