Care coordination

Case Managers coordinate comprehensive assessment and treatment plans and guide care provided by services both internal and external to the Centre.

Case Managers work with veterans, their family and carers and Centre staff to provide individually-tailored assessment, support and links to programs. Case Managers assist veterans navigate health and wellbeing services and facilitate continuity of care.

Care coordination involves development of comprehensive care plans that ensure:

  • Coordinated case management service (across all health sectors and Concord Hospital)
  • Connection and referral to external services
  • Liaison between patients, GPs, other hospitals, community and other services
  • Care management plan development

Following discharge from the Centre, Case Managers are the point of contact for veterans and community care providers. At set time intervals following discharge, Case Managers contact veterans and community care providers to ensure goals are being met and to provide support if required.
