Intake process

The intake process assesses a veteran’s suitability for treatment at National Centre for Veterans’ Healthcare. If other services are considered more appropriate, the Centre will assist in facilitating access or referrals.

National Centre for Veterans’ Healthcare Case Managers conduct a preliminary 20 minute phone interview to form an initial assessment. Veterans are asked to complete a series of online surveys.

Assessment questions may include:

  • A brief medical, mental health and psychosocial history, including current medications and treatment
  • Demographic information
  • Risk assessment *
    * Immediate management of identified high risk issues will occur if required.

If a veteran is assessed as suitable for treatment at National Centre for Veterans’ Healthcare, the Case Manager will schedule a second phone interview.

This interview will consist of a number of additional health and lifestyle questions and takes about 30 minutes.

Following this second interview, veterans are triaged and the first face-to-face appointment is scheduled.

*Please note, in circumstances where there may be a significant delay between referral and first appointment, Case Managers can offer interim management strategies.

Veterans are encouraged to contact their allocated Case Manager to advise of any changes in health status or personal information.
