Telehealth is the delivery of healthcare at a distance using Information Communication Technology (ICT). Telehealth connects clinicians to a patient, carer or any person responsible for providing care to a veteran. Telehealth can be used assessment, intervention, consultation, education and supervision.
National Centre for Veterans’ Healthcare Case Managers aim to link with a veteran’s GP prior to discharge from the service, to ensure all parties are aware of recommended ongoing management plans. Telehealth services are particularly useful in these follow-up discussions and reviews to promote continuity of care.
Telehealth is not used for medical assessments as these are undertaken face-to-face.
