Have Your Say
The Sydney Local Health District (SLHD) welcomes suggestions, compliments and complaints as part of our commitment to provide a high quality service. Tell us what we’re doing well; we appreciate hearing from you. If you have a concern, we want to know about it. We value open and timely communication and we understand the importance of resolving matters promptly; it benefits our ongoing relationship with you and helps us improve our services.
If you would like to provide us with your feedback, whether it be a suggestion, compliment or a complaint, please select from the Feedback Assist icons on the right side of this page.
What is Feedback Assist?
Feedback Assist is a new feedback system available now on our website, which aims to give our patients and the public a consistent contact point for lodging either a suggestion, compliment or a complaint.
How does it work?
The feedback icons consist of a happy face, a sad face, and a lightbulb. By clicking the relevant icon you can submit your feedback.
Once your feedback has been submitted, an automatically generated reference number will be issued to you. You have the option to receive a written response to your feedback, a phone call or you may remain anonymous.
Who can use Feedback Assist?
Anyone can use the icons to provide feedback, including our patients, members of the public, volunteers and staff.
What happens when I provide feedback?
All information received will be reviewed and provided to the appropriate department or staff member for their information and action.
All complaint matters will be assessed and handled in accordance with the NSW Ministry of Health, Policy and Guidelines for complaint handling.
Within two business days, you will receive an acknowledgement, indicating who will be responding to your feedback. You will be advised of a contact person should you need further assistance in the interim.
How will my privacy be protected?
We are committed to treating your personal and health information in accordance with NSW privacy law and regulations. For more information, please click here.
