To assist residents and their families understand the progression of chronic disease or end of life care. It is recommended that these information sheets be used as part of a discussion rather than being distributed in isolation.

NSW Palliative Ambulance Care Plan

A new service has been introduced by NSW Ambulance to avoid unplanned transfers to hospital during end of life care.  In consultation with their palliative patients and carers or families, a treating doctor can submit a treatment plan to NSW Ambulance. If Triple Zero (000) is called, the approved plan authorises the ambulance officer to follow the doctors prescribed orders.
