Updating or changing documents in the medical record:

1. Advance Care Directive

“How can I update my Advance Care Directive?”
You can update or remove (also known as revoking) your own Advance Care Directive at any time. Please send any new documents as per the submission options. Include a note with:

  1. your name and contact details
  2. which document is to be updated or removed.

"Can I update someone else’s Advance Care Directive?"
You can only update or remove another person’s Advance Care Directive with their written permission. Include this permission letter and send as above.

“How often should I update my Advance Care Directive?"
It is highly recommended that you update your Advance Care Directive annually, or if there is significant change in your medical condition.

2. Advance Care Plan

"How can I update  an Advance Care Plan for someone else?"
You can only update an Advance Care Plan for someone else if they have given you permission or if you are the Person Responsible.
Please send any new documents as per the submission options. Include a note with:

  1. Your name and contact details.
  2. Your relationship to the person.
  3. Details of which document is to be updated.
