Your Wishes

Document your wishes

There are two types of documents you could consider. Only one type of document should be completed.: 

  1. Advance Care Directive (ACD):
    The preferred legal document written by you about your own healthcare decisions.
  2. Advance Care Plan (ACP):
    A plan usually written for you and used as a guide for your healthcare decisions.

Both these documents may include the following:

  • Identification of your Enduring Guardian (if formally appointed) or Person Responsible so they can make decisions on your behalf.
  • Directions about healthcare in the event of a life-threatening illness or accident.
  • A record of your wishes, including cultural, spiritual and social needs that are important to you.
  • Any combination of the above.
