Important Notice for Parents and Guardians
From 1 July 2025, you must sign a Child Dental Benefits Schedule Bulk Billing Consent Form each time your child (aged 0–17) visits a public dental clinic in NSW. This form allows us to bulk bill Medicare for your child’s treatment. You will not have to pay anything, even if your child cannot get the benefit or has used it up.
Please note, you must sign the form before your child can get treatment.
