Our Research

Chronic & Complex Care
An Equity-Focused Review of patients registered to Chronic & Complex Care CNCs was progressed in 2016. This research examines the demographics of our patient cohort through a health equity lens. Referral sources and intervention profiles are also examined. This research will inform further inquiry into the effectiveness of the program

ACC&R
This research aims to trial and evaluate on patients attending NSW hospital clinics with advanced illnesses identified as potentially having supportive and palliative care needs, a model of ACP that seeks to:
(i) increase the uptake of ACP by patients; and
(ii) encourage NSW Health clinical services and GPs and other health professionals providing care for the patients to incorporate ACP into routine care. This study seeks to determine if this model of ACP will reduce acute health resources utilisation; improve the quality of care for patients and caregivers / family; and result in improved understanding and uptake of ACP by health professionals.

Key Achievements

Chronic & Complex Care:
Collaboration with the SLHD Health Equity and Research Development Unit to receive mentorship for beginning researchers.  - Increasing confidence with research process through 'learning by doing' model.

CRGH Endocrinology & Metabolism:
1. Asia and Latin America Observational Study with Eli-Lilly being the project Sponsor.
2. International study with a UK Hospital sponsoring the research. We have identified family members of Paget's patients with a genetic predisposition to the disease and initiated medical therapy to hopefully prevent Paget's developing in the future.

ACC&R
1. Translational research grant scheme funded by the MoH.
2. Collaboration between SLHD and SESLHD.
3. Education/workshops days x2 held at Concord
4. Having 10 + outpatient clinics involved in the study and actively recruiting patients for the study.
5. Uploading completed ACP/ACD onto eMR
6. Educating patients/families about ACP.
7. Educating staff about advance care planning and directives.
8. Educating GP's at HPN meetings about ACP and the project.

ACC&R - CNC Annie Hepworth - Dementia Delirium SLHD:
Building a substantial research agenda in my field.

Hospital in the Home:
Ethics and site approvals achieved.  Data collection underway.

Amputee & Rehabilitation:
Studying effort/reward balance and commitment between generations of nurses.
