The Department of Neurosurgery at Royal Prince Alfred Hospital is focused on cerebrovascular surgery, stereotactic radiosurgery (SRS), epilepsy surgery, spine surgery, acoustic neuroma and transsphenoidal pituitary surgery and general and trauma neurosurgery. There is a strong emphasis on endovascular treatment of cerebrovascular disorders.

The Department's procedural capacity includes intraoperative MRI, intraoperative stereotaxic navigation, minimally invasive surgical EEG monitoring, access to hybrid operating room and comprehensive anaesthetic services.

The Department of Neurosurgery enjoys close liaison with Anaesthetics, other surgical departments (especially Otorinolaryngology, Head & Neck Surgery), Radiology, Neuropathology, Medical Oncology, Radiation Oncology, Rehabilitation Medicine and Neurology.  Complex conditions are managed in a collaborative fashion through multidisciplinary team meetings.

RPA teaches at undergraduate and postgraduate level for medical, nursing and allied health education, and provides advanced training in Neurosurgery leading to FRACS.  The Department of Neurosurgery has two accredited registrar (SET) positions in addition to other resident medical staff. Recent research initiatives include the establishment of "Brainstorm" in with the Department of Neuropathology to analyse the behaviour and treatment of primary brain tumours and involvement with the Royal Prince Alfred Institute of Academic Surgery.
