SLHD Community Nursing Referral Form 

For palliative care medical home visits for NON AMBULATORY patients in the SLHD area, please fax medical referral to 97678064

All patients referred for a medical home visit must be known to the SLHD community palliative care nurses. If not known, please also make a community nursing referral:
