NSW End of Life / Integrated Care Pathways - 25 February 2010

This interactive symposium will review various applications of End of Life Care Pathways in NSW, and consider what steps are needed to extend their uptake in a range of care settings.

Aim
To share our experiences and understanding of End of Life Care and Integrated Care Pathways by
 
o         exploring the evidence and applications of End of Life Care Pathway initiatives
in a range of rural, regional and metropolitan settings
o         highlighting key outcomes of, and challenges to their development
o         making explicit the factors that improve sustainability in End of Life Care
Pathway projects
o         exploring ways of promoting and supporting the uptake of End of Life Care Pathway initiatives in NSW
