Verification

NSW Health can now accept scanned documents from clinical facilitators for verification purposes. Review the requirements below and return documents in pdf format for processing. Note evidence of the current ‘flu vax’ is now an annual requirement. Flu season is traditionally between 1 June and 30 September each year. As soon as the flu vax is available at your local pharmacy or from your GP, have this vaccination administered and return this evidence to the SLHD Clinical Placement Unit; we can update your ClinConnect file. This will negate the requirement of having to present this evidence on each and every subsequent clinical placement you attend. Ideally this evidence should be recorded on your vaccination card as this document serves as your permanent immunisation record as an adult.

Review the mandatory requirements below and ONLY return the documents noted, in the order requested. Files containing multiple irrelevant documents will be returned and not processed.

    1. SLHD staff must also complete the 'SLHD Compliance Summary Sheet for Clinical Facilitators'

    2. An Australian National Police Certificate; the expiry date of this document is three years from the date of issue.

      If you are not an Australian citizen or hold permanent residency, you must submit a statutory declaration, signed in front of a Justice of the Peace or other legal office confirming you have no criminal convictions prior to entering Australia.     3. A signed NSW Health Code of Conduct form.

    4. Evidence of NSW Working with Children Check - the WWCC 'number' issued on documentation provided by the RMS must be provided; not a copy of the application receipt number indicating that an application to obtain a WWCC has been completed. The WWCC number will be validated by SLHD Human Resources and the clinical facilitators profile will only be updated to compliant once this has occurred.

    5. Proof of AHPRA Registration

    6. All immunisation history including serology and a completed Undertaking/Declaration Form and Tuberculosis (TB) Assessment Form must be submitted for review by Staff Health. Immunisation cards must be completed correctly; that is Medical Officers must sign each vaccination and provide their practice stamp and provider number. Hepatitis B and Rubella serology must be recorded as numerical values - not 'positive' or 'negative'. If a history of age appropriate hepatitis B vaccinations is unavailable to be recorded on the vaccination card when being assessed by your GP or accredited immunisation provider, complete Hepatitis B Vaccination Declaration and submit this with other documents for review

    7. A blank 'Vaccination Record card for Healthcare Workers and Students' is supplied here.

    8. Present a signed 'Attachment 1 and 2 for Facilitators'

All documentation presented to SLHD staff for verification purposes must be scanned and emailed in pdf format to slhd-cpu@health.nsw.gov.au
