Sydney Local Health District eMR Access Guide

Returning for another placement in Sydney Local Health District?

  • Access Period: Nursing and Midwifery students are granted access to the SLHD electronic Medical Record (eMR) system for three years; however, access will expire if you have -
    • not used the SLHD eMR system for six consecutive months, or
    • undertaken a placement in another Local Health District (LHD).

    If you're unsure about your access status or if any of your personal details have changed or you are unable to gain access to the eMR using your assigned log-in credentials, please contact your education provider for assistance and advice - DO NOT call the Statewide Help Desk or approach IT services.

Starting a placement at SLHD for the first time?

To gain access to the SLHD electronic Medical Record (eMR) system, students must follow these steps:

Timeline & Deadlines

  • Complete the following at least 10 working days before your clinical placement begins:
    1. Access and complete the 'Overview of the eMR for Nursing Students' module via My Health Learning.

      eMR Access

      • To find this module quickly, type the course code: 182216601 into the search engine.

    2. Submit a correctly completed eMR User Access Request Form to your education provider. An example of how this form should be completed is located here

  • Education providers must submit:
    • All completed UAR forms.

    • Placement Allocation Report (confirming eMR training completion) No later than 7 working days before placement starts.

    • An example of how the Placement Allocation report showing that the student has completed eMR training is located here

    • Please do not submit application forms for eMR access where no evidence has been supplied that the student has completed the required eLearning module.

      Late submissions will not be prioritised.
      Students not completing the online module and submit their paperwork will not be given eMR access
      and may not be able to proceed with their placement.

eMR Username

  • Your eMR username is your StaffLink number, ending with a capital 'S' (e.g. 6xxxxxxS)

Student Instructions (Steps 1-3)

  1. Complete Training: Log into My Health Learning and complete the 'Overview of the eMR for Nursing Students' module.

  2. Fill Out the Access Form

    • Download the eMR User Access Request Form.

    • Complete all fields clearly, including:
      • Placement dates.
      • Your signature on page 2.

    • If the e-sign option is missing:
  3. Submit to Your Education Provider

    • Students employed by NSW Health must still apply and include their current StaffLink number.

    • A new student StaffLink number will be issued to avoid login conflicts with employment profiles.

Education Provider Instructions (Steps 4-7)

  1. Review Forms
    • Ensure all details (StaffLink number, email) are legible and accurate.

  2. Verify Identity
  3. Prepare Reports
    • Collate all forms and prepare a Placement Allocation Report for each facility. (An example is provided here).

  4. Submit to SLHD Clinical Placement Unit*
    • Email completed forms and reports to:
      slhd-cpu@health.nsw.gov.au

    • Students do not need to submit their Certificate of Achievement if the allocation report is included.

    • Please note the SLHD Clinical Placement Unit cannot reset passwords or establish eMR access. Education providers can contact the eMR team directly to discuss issues relating to a student's eMR access.

SLHD & DHSES Responsibilities (Steps 8-9)

  1. Following submission of application forms and the placement allocation report to the SLHD Clinical Placement Unit
    • The CPU will forward all documentation to DHSES, for account creation.

  2. Login Details Distribution
    • DHSES will return login credentials to the education provider, who will then pass them on to students.

Students should check their emails for login details and for the assigned temporary password - if no email has been supplied with this information, the student is to contact their education provider.
