Referrals and Contact

We are no longer accepting generic referrals. All the referrals should be address to Dr Ted Wu, Director, Royal Prince Alfred Hospital Diabetes Centre.

Referral letters must include at least:

• the reason for the referral
• minimum pathology set collected within the last three months, including:
• HbA1c
• Serum biochemistry including LFT and creatinine
• Lipid levels, HDL cholesterol
• Full blood count
• Urinary micro albumin, protein, creatinine and albumin excretion rate
• ECG

Referral letters can be emailed to slhd-rpadiabetes@health.nsw.gov.au 

If the appointment is urgent, health practitioners should call the Diabetes Centre on 02 9515 5888.

Patient appointments

Once referral has been received, the Diabetes Centre will contact the patients to arrange their first appointment.

Please bring the following to your appointment:

• your medication
• copies of any recent blood tests
• your blood glucose meter
• your blood glucose record book (if you use one)
• Downloaded copy of glucose result if you have one
